Vision: Leading Mississippi toward a diabetes free tomorrow
Mission: To serve as a unified voice to reduce the impact of diabetes through improved policy, prevention, and management of diabetes and its complications for all Mississippians.
GOAL: Advocate for policy changes that support diabetes prevention and management in Mississippi.
Mobilize Coalition members and their networks to coordinate and conduct advocacy activities.
Educate and motivate policy makers and community leaders to promote public policies and programs that support diabetes prevention and control.
Improve reimbursement for comprehensive diabetes prevention, management, and diabetes self-management training.
GOAL: Promote prevention, education, and utilization of health care services to reduce diabetes-related health disparities.
Encourage the identification of children, adolescents, and adults with pre-diabetes and diabetes through screening and awareness.
Promote effective strategies and interventions to increase preventive behaviors among patients with or at risk for diabetes to reduce health risks.
Promote professional education opportunities on risk factor assessment, behavior change counseling skills, diabetes prevention and control, and cultural competency.
GOAL: Collaborate with health systems and providers to ensure evidence-based care is provided to all people with diabetes to reduce the risk of diabetes complications.
Improve the delivery of comprehensive diabetes care to all Mississippians.
Enhance partnerships and communication with payers, providers, health and community organizations, and other relevant partners to support standards of care for diabetes.
Assure community access to reliable, accurate, and culturally-relevant patient education resources and information.
Promote cultural competence of health care professionals.
